Services

Three focused areas of care.
One consistent standard.

Empire Medical Wellness provides private, physician-led outpatient care in obesity medicine, addiction medicine, and adult mental health. You may seek care in one area or more than one. Every service begins with a focused medical evaluation and a treatment plan designed around your needs.

Obesity Medicine

Addiction Medicine

Adult Mental Health Care

Medical weight management built for lasting progress.

Obesity is treated at Empire Medical Wellness as a chronic medical condition requiring individualized assessment, treatment, and long-term planning.
Care begins with a review of your weight history, health risks, previous efforts, daily routines, current medications, and goals. Your treatment plan may include nutrition and activity goals, sleep and routine changes, review of relevant medical factors, and anti-obesity medication when clinically appropriate. Long-term maintenance is considered from the beginning rather than treated as an afterthought.

Your first obesity medicine appointment

The initial 50-minute appointment focuses on your weight history, health concerns, previous approaches, current medications, goals, and appropriate treatment options. You will leave with a clear understanding of the recommended next steps and follow-up plan.

Your obesity medicine care may include

Medical Assessment

Review of your weight history, current health, medications, previous treatment, laboratory information, and risk factors.

Personalized Treatment

A structured medical plan designed around your health, schedule, preferences, and goals.

Medication Management

Evaluation and management of anti-obesity medication when it is clinically appropriate for your care.

Weight Maintenance

A deliberate strategy for protecting progress, responding to setbacks, and supporting long-term maintenance.

“Weight loss matters. Maintaining it matters just as much.”

Dr. Chhatpar’s obesity medicine approach is informed by dual board certification and personal experience losing and maintaining 110 pounds through diet, exercise, structure, and long-term lifestyle changes.

Confidential medical care for change, treatment, and recovery.

Adults seek addiction medicine at different stages. Some are beginning to question their substance use. Some want to reduce risk or stop using. Others are returning to treatment, strengthening recovery, or seeking continued medical support.
Care begins with a confidential assessment of your current situation, medical risks, previous treatment, medications, and goals. The treatment plan is based on what is clinically appropriate for your needs and stage of change.

Your first addiction medicine appointment

The initial 50-minute appointment focuses on your current concerns, medical history, substance use history, previous treatment, medications, goals, and immediate clinical needs. You will receive a clear explanation of treatment options and the recommended next step.

Addiction medicine care may include

Care Coordination

Communication with therapists, recovery professionals, and treatment programs when clinically useful and authorized.

Coordination across levels of care

When detoxification, residential treatment, partial hospitalization, intensive outpatient treatment, or another level of support is clinically appropriate, Empire Medical Wellness can help coordinate the next step with trusted treatment partners. Outpatient medical care can continue or resume when it is clinically appropriate to do so.

Thoughtful evaluation and medication management.

Empire Medical Wellness provides mental health evaluation and medication management focused on understanding symptoms, improving daily functioning, and developing a clear treatment plan.
Care begins with a comprehensive review of your symptoms, medical history, current functioning, previous treatment, and current and past medications. Medical conditions, sleep, substance use, lifestyle, and other relevant factors are considered when they affect diagnosis or treatment response.
