What the initial evaluation includes

The initial evaluation is a 50-minute medical appointment designed to establish a clear understanding of your concerns and determine an appropriate treatment plan.

Primary Concern

A focused discussion of the reason you are seeking care and what you would like to change.

Medical & Treatment History

Review of relevant medical history, previous treatment, current medications, and available records.

Clinical Assessment

Evaluation of the factors that are most relevant to diagnosis, treatment, safety, and follow-up.

Clear recommendations, treatment options, and the proposed schedule for continued care.

Private-pay care

Empire Medical Wellness is a private-pay medical practice and does not bill insurance directly. Payment is due according to the financial policy provided during scheduling. Fees are confirmed before an appointment is finalized.

Laboratory testing, pharmacy costs, imaging, treatment programs, and services provided by outside clinicians or facilities are billed separately by those organizations.

An itemized receipt is available through the patient portal.

Frequently asked questions

Do I need a referral?

No referral is required to request an appointment.

Can I schedule care for only one service?

Yes. You may seek obesity medicine, addiction medicine, or mental health care as a focused service.

Is there an in-person office in New York?

New York appointments are currently provided through an in-person office as well as telemedicine. In-person appointments are also available at the practice’s New Jersey location.

Does the practice prescribe medication?

Medication may be included when clinically appropriate. Treatment decisions are made after evaluating your history, current condition, safety considerations, and goals.

What information is needed before the first visit?

Please provide an updated medication list, pharmacy information, relevant laboratory results or records, and contact information for other clinicians involved in your care.

What is the cancellation policy?

Appointments canceled or rescheduled with less than 24 business hours’ notice are charged the full appointment fee. The complete financial policy is provided before scheduling is finalized.

Are forms, letters, and extensive record reviews included?

Non-routine forms, letters, reports, extensive record review, or clinical work requested outside a scheduled appointment may be billed separately. The applicable fee is communicated before the work begins whenever possible.

What happens if I need more support than outpatient care provides?

When another level of care is clinically appropriate, the practice can assist with coordination to detoxification, residential treatment, partial hospitalization, intensive outpatient treatment, or another appropriate service.
